name change Address, name change Your address change You are responsible for keeping your address current. The Board requires written verification of an address change. The easiest way to update your address is to email the Board with the change. Include "Address change" in the subject header. You can also fax or mail your change. FYI…When contacting the Board by email, do not use ALL CAPS in the subject header. Our email filter interprets ALL CAPS as junk mail and we may not receive your communication. The Board cannot accept address changes by telephone; we need written or printed verification of your address change. Your name change Most frequently, name changes include firms with a Certificate of Authorization. On occasion, name changes include individuals. Individual name change The easiest way to update your individual name change is to email the Board with the change. Include "Name change" in the subject header. You can also fax or mail your change. Firm name change requires a new application A firm name change requires a new application [ENG 1021-N] in order to maintain a Certificate of Authorization. email addresses have changed REMINDER…The Board has changed its email addresses. Our @pes.ohio.gov email addresses are current and available. Our previous @mail.peps.state.oh.us email addresses have been discontinued and will bounce back. If you email the Board using links on our web pages, the change should be seamless.
